A credit card on file is required to schedule all appointments. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.  The card will only be charged in cases in which Eternal Dermatology is not given the appropriate notice of cancellation or if you do not show up for your appointment.

LATE CANCELLATION AND NO-SHOW POLICY

At Eternal Dermatology and Aesthetic Center, we are committed to providing exceptional care. To achieve this objective, we must develop a cooperative relationship with our patients. In fostering this relationship, we have developed a comprehensive cancellation and no-show policy that will assist us in streamlining the appointment process and provide greater coordination of patient flow within our office.

 Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, patients who do not show up for their appointment, or who do not call to cancel within the appropriate time period before the appointment time will be considered a “NO SHOW.”

Below are all of our types of appointments and the window appropriate for each type, any violation of the appropriate window results in a No Show Charge:

Cosmetic Appointments

It is the patient’s responsibility to cancel or change his/her appointment at least 48 hours (2 business days) prior to the appointment time and date. Otherwise, their $200 cosmetic deposit will be forfeited.

Example: An appointment scheduled on a Monday at 8:00 AM needs to be rescheduled by 8:00 AM on the preceding Thursday. A missed cosmetic appointment results in the cosmetic deposit being forfeited.

Medical Visits

It is the patient’s responsibility to cancel or change his/her appointment at least 24 hours (1 business day) prior to the appointment time and date, otherwise, a $50 fee* will be billed to the patient’s account.

Example: An appointment scheduled on a Monday at 8:00 AM needs to be rescheduled by 8:00 AM on the preceding Friday. A missed office visit results in a $50 fee.

Surgical appointments

It is the patient’s responsibility to cancel or change his/her appointment at least 48 hours (2 business days) prior to the appointment time and date, otherwise, a $75 fee* will be billed to the patient’s account.

Example: A Surgery scheduled on a Monday at 8:00 AM needs to be rescheduled by 8:00 AM on the preceding Thursday. A missed surgical appointment results in a $75 fee.

“No Show” fees will be billed to the patient. This fee is not covered by insurance cannot be submitted to insurance for reimbursement, and must be paid in full prior to your next appointment.

Our office may provide a reminder email and/or text in advance of your scheduled appointment. These reminders are issued as a courtesy. If for any reason you do not receive these reminders, it is still your responsibility to keep track of, and attend your scheduled appointment, or cancel within the appropriate time frame.

Please help us serve you better by keeping scheduled appointments.

Repeated missed appointments may result in you being discharged from the practice.

Fees and Payments

Payment for office visits and services such as biopsies, excisions, and cosmetic services are expected at the time of the appointment. Many insurance plans require co-payments, co-insurances, and/or deductibles. We will collect these payments at your appointment.

Exclusions:

Before your appointment, you should confirm with your insurance company if you have exclusions on your policy. For example, you confirmed that you are in-network with Eternal Dermatology. Still, you may only be able to be seen for acne because other conditions are excluded from your policy. We are unable to determine this by looking at your insurance card.

Please remember to bring your Insurance card and ID with you to every visit. Failure to present both your Insurance card and ID upon arrival to your visit will require you to reschedule your appointment.

If you have no insurance or Eternal Dermatology does not accept your insurance, we will gladly see you as a “Self Pay” patient, and you have the option to submit the claim to your insurance company on your own. New patient consultations are $300. Established patient visits are $200. Any procedures performed, (for example, skin biopsy) would be an additional cost. Pricing would be provided to you upon evaluation.

If you do not have Insurance

If you have no insurance or Eternal Dermatology does not accept your insurance, we will gladly see you as a “Self Pay” patient, and you have the option to submit the claim to your insurance company on your own. New patient consultations are $300. Established patient visits are $200. Any procedures performed, (for example, skin biopsy) would be an additional cost. Pricing would be provided to you upon evaluation.

What is the cost of a “Self Pay” visit? ( For those that do not have Insurance that Eternal Dermatology accepts)

For “Self-pay” visits, new patient consultations are $300. Established patient visits are $200. Any procedures performed, (for example, skin biopsy) would be an additional cost. Pricing will be provided to you upon evaluation.

What is the difference between a medical and a cosmetic visit?

Your insurance usually covers medical services because they are considered medically necessary. Examples include warts, acne, skin cancer surgery, rashes, and skin infections. Before your appointment, you should confirm with your insurance company if you have exclusions on your policy. For example, you confirmed that you are in-network with  Eternal Dermatology, but you may only be able to be seen for acne because other conditions are excluded from your policy. We are unable to determine this by looking at your insurance card.

 Cosmetic procedures are considered elective and are not covered by your insurance company because they are considered not medically necessary. Examples of such procedures are Botox, chemical peels, microdermabrasion, Restylane, skin tag removals and removal of benign moles, earlobe repair, and more.

What services are usually covered by insurance?

The following conditions are usually covered by insurance:

  • Acne
  • Skin cancer
  • Warts
  • Cysts
  • Rashes

 However, there may be exclusions in your specific plan with your insurance. Before your appointment, you should confirm with your insurance company if you have exclusions on your policy.

What is a Cosmetic Consultation, and how much does it cost?

At the Cosmetic Consultation ($200), we evaluate your skin, discuss your concerns, and then discuss the best treatment options to achieve your desired goals. You must have a consultation performed before having a cosmetic procedure performed at Eternal Dermatology. This is important because we need to first determine which procedures are right for you, will give your desired outcome, and that you are a good candidate.

If you have a consultation performed, but later want to have a different procedure performed (that we have never evaluated you for, or discussed with you,) you will need to set up a consultation before we perform that procedure.

At Eternal Dermatology, there are a few different types of Cosmetic Consultations:

  • (Cosmetic Consultation) Botox, Fillers, Microneedling, Skin Rejuvenation, Discoloration, Scars
  • Mole Removal Consultation
  • Earlobe Repair Consultation
  • Laser Consultation
  • Chemical Peel Consultation
  • O-Shot PRP Consultation

All of the Cosmetic Consultations are priced at $200. If you have a procedure that we discussed during your consultation performed within 30 days of your consultation, then there is a $150 credit to the cost of your procedure.

If you choose not have a procedure done, or it is done more than 30 days after your consultation, the cost of the consultation remains at $200, and there is no credit given to the cost of your procedure.

How do I schedule a Cosmetic Procedure if I already had a consultation done?

To schedule your cosmetic appointment, please call us, or you can book online. There is a $200 deposit to hold your appointment. If you do not show up for your appointment, or fail to give 48 hours (2 business days) cancellation notice, or if you show up for your appointment, but decide to not have the procedure done, then your deposit will be forfeited.

What is the difference between an “Acne visit (Covered by Insurance)” and an “Extensive Acne Evaluation?”

The “Acne visit (Covered by Insurance)” is a medical visit where we diagnose, discuss and prescribe medical treatments for your acne. This visit is usually covered by insurance.

In the “Extensive Acne Evaluation,” we discuss both medical and cosmetic treatment options (chemical peels, lasers, Sebacia) for your acne. 

The cost of this consultation is $300. And if you choose to move forward with a cosmetic treatment for your acne  within 30 days of your consultation, then you receive a $150 credit to the cost of your procedure.

What is the difference between an “Hair Loss / Scalp Condition visit (Covered by Insurance)” and an “Extensive Hair Loss Consultation?”

The “Hair Loss/ Scalp Condition visit (Covered by Insurance) is a medical visit where we diagnose, discuss and prescribe medical treatments for your hair loss or scalp condition. This visit is usually covered by insurance, but some insurance plans may exclude (do not cover) hair loss.

In the “Extensive Hair Loss Consultation,” we discuss both medical and cosmetic hair restoration options (including PRP, Kerafactor, and Hair Transplantation).  If your insurance plan does not cover hair loss, you may opt to be seen under this appointment type. The cost of this consultation is $300. If you choose to move forward with a cosmetic hair restoration treatment within 30 days of your consultation, then you receive a $150 credit to the cost of your procedure. 

Facials and Aesthetician services

Aesthetician Services: Skincare Regimen Consultation

If you would like to discuss and come up with a skincare regimen, please book a “Skincare Regimen Consultation” with our aesthetician.

During your 15-minute skincare consultation, our Esthetician will begin by asking you to explain the goals you have for your skin. She will then help to clarify your current product regimen, as well as assess what is working and what may need to change.

The $50 payment will cover the cost of your consultation. (additional cost for treatment/products)

Aesthetician Services: Facials

A $50 non-refundable deposit is required to book a facial with our aesthetician. This deposit goes towards your facial. It also includes a comprehensive skin analysis and personalized medical-grade skin care recommendations for your skincare regimen.
